At Cosmetic Laser MD and the UltraShape Center, we pride ourselves in offering the best care for our patients. Please make yourself aware of our practice policies and procedures described below. Our policies have been designed to ensure you enjoy a truly peaceful and relaxing visit with us.

CONSULTATION – Cosmetic Laser MD and the UltraShape Center are dedicated to helping you achieve your aesthetic goals. We offer complimentary consultations to better understand each individual’s needs. Based on a private skin and/or body analysis and conversation, we partner with you to design a treatment plan that will best meet your expectations.

SCHEDULING – Cosmetic Laser MD and the UltraShape Center are always buzzing, so appointments are usually necessary; but when time permits, walk-ins are welcome. Please call ahead and we will do our best to schedule you up with any of our premier services. For your convenience, we recommend scheduling your next treatment before leaving our facility. Due to the medical nature of many of our treatments, appointments can sometimes exceed their scheduled time. Please be patient and know that you will also receive the same exceptional service and personal attention.

CANCELLATION POLICY – A scheduled appointment at Cosmetic Laser MD and the UltraShape Center reserves the time of our professionals. Consultations with Dr. Tanne are complimentary. Out of respect for the doctors time, we do require a credit card to secure all appointments. Cancellations are accepted up to 24 hours prior to your appointment without incurring a charge. Cancellations with less than 24 hours or no-shows require a $50 service fee which will be charged to your credit card. Thank you for your consideration.

SCHEDULING FOR ULTRASHAPE CENTER, PRP HAIR RESTORATION AND THERMIVA – All consultations at UltraShape Center and Cosmetic Laser MD are complimentary. Out of respect for the doctor and technician, a credit card is used to secure all consultation appointments. When booking your treatments, a 50% non-refundable deposit is required. If you are securing the package pricing, 50% of the package cost is required at the time of scheduling your appointment with the balance due at the time of your first appointment. If you are paying per treatment, 50% of the single treatment fee is due upon booking with the balance due at the time of your appointment. THERE ARE NO REFUNDS FOR ULTRASHAPE, PRP HAIR RESTORATION, AND THERMIVA.

“NO SHOW POLICY” FOR ULTRASHAPE, THERMIVA AND PRP HAIR RESTORATION – To not show up for a scheduled treatment shows a lack of respect for our practice and our time. There is a PENALTY FEE 50% of the treatment cost (equal to your pre-paid deposit for a single treatment)for all “no show” appointments. NO EXCEPTIONS!

CANCELLATION POLICY FOR ULTRASHAPE, THERMIVA AND PRP HAIR RESTORATION – An appointment for these procedures occupies a large block of time in our schedule. Out of respect for the doctor’s and technician’s time, there is a cancellation charge of 10% of the treatment cost if you cancel less than 24 hours prior to your appointment.

ARRIVAL – Please arrive 10 minutes prior to your appointment, so you’ll have plenty of time to unwind, check out our CLMD Advanced Skin Solutions (skincare), and give us the 411 on what’s new in your life (aka paperwork). As we are a medical spa, we require that all new patients fill out a medical/skincare history and treatment consent forms. Paperwork and consent forms must be updated annually. There’s nothing fashionable about being late, you’ll just deprive yourself of the full treatment time.

PHOTOGRAPHY – No treatments (laser, injectable, peel, body contouring, etc.) will be done without pre and post-treatment photography. Patients who refuse photography will not be treated. All photographs are private and covered by HIPPA laws. Post-treatment and all follow-up visits do include photography.

PAYMENT – We accept cash, personal checks, Visa, Mastercard, American Express, Discover and Care Credit. All treatments/products must be paid for at the time of treatment/purchase. There are no refunds for the services performed by Dr. Tanne and the staff of Cosmetic Laser MD and the UltraShape Center. Gift certificates are not refundable or transferable. Skincare products may be returned within 7 days of purchase with a receipt, for credit toward skin care. There are no discounts when using Care Credit. We do accept personal checks as a form of payment, however, if a check is returned for insufficient funds, a fee of $30 will be added to the amount due.

PACKAGE PRICING – NOT APPLICABLE TO ULTRASHAPE TREATMENTS – Package pricing represents a significant discount off of single treatment prices, and payment for a package must be made at the time of the first treatment. If a package is discontinued at the request of the patient, treatments received will be charged at the individual (non-package discount) price. If you elect not to complete a treatment package, the unused portion will be credited to your account or refunded. EXAMPLE: If a treatment is normally $550, and a 5 treatment package is priced at the discounted price of $2,500 (a $250 package discount), and you only use 4 treatments, you will be refunded or credited $300 which represents the remainder after 4 treatments at $550 each are deducted. ALL PACKAGES EXPIRE ONE YEAR FROM PURCHASE DATE.

SIMPLY BEAUTIFUL REWARDS POINTS – Simply Beautiful Reward Points give you an easy way to earn points and save money on services. You will earn 1 Rewards point for each $1 you spend. You can earn points several ways that can be redeemed for any of our aesthetic services, with the exception of UltraShape treatments. Once you’ve reached the 2000 point level, you may use your points toward the service of your choice. Reward Points are not valid for purchase of products or for UltraShape treatments. Rewards Points ARE NOT transferable and will expire after an eighteen month period.

FOR YOUR ENJOYMENT – For their safety, and for the courtesy of other guests receiving relaxing services, children under 13 years of age are not permitted. Please turn off cell phones, pagers, Blackberries, iPhones, and iPods. This is your time to relax and be in the moment. Inform our front desk staff if you have any special needs or concerns to allow us to ensure your time spent with us a memorable experience.

VALUABLES – We are not responsible for lost or stolen items and encourage you to leave valuables at home.

For the comfort of our guests, Cosmetic Laser MD/UltraShape Center is a Non-Smoking facility.

We wouldn’t be the trendsetters that we are if we weren’t constantly expanding our services. Although Cosmetic Laser MD and the UltraShape Center make every effort to keep its website updated, please note that prices and services are subject to change at any time. We love having you as a patient, but we reserve the right to refuse service at any time, to anyone, for any reason.
