Cosmetic Laser MD is delighted to introduce

“Simply Beautiful Rewards,”

our new reward loyalty program.

We value each and every patient and we are thrilled to reward you with an easy way to earn points and save money on services. You can earn points several ways that can be redeemed for any of our aesthetic services.

THERE ARE LOTS OF WAYS TO EARN POINTS: 
1. Simply join SIMPLY BEAUTIFUL Rewards (it’s free, and takes about a minute). We’ll start you off with 200 points immediately! Sign up online or at our front desk. You will receive a SIMPLY BEAUTIFUL Rewards card with your Reward Card Number. Be sure to use this number every time you check out so that we can issue points.

2. After completion of services, present your card, and points will be automatically awarded to your account. You will earn 1 SIMPLY BEAUTIFUL Rewards point for each $1 you spend. (We will keep your number on file if you forget your card!)

3. When you purchase a Gift Certificate for a friend, we will issue DOUBLE the amount of the certificate.

4. If you visit us during your Birthday Month (and we hope that you do), tell us at checkout, and we will issue you an extra 100 SIMPLY BEAUTIFUL Rewards points.
5. If you refer a friend (who treats over $500) you will automatically earn 500 SIMPLY BEAUTIFUL Rewards points.

6. Events and Special Promotions – each month, we will be announcing additional ways to earn even more points – watch your email for more info to earn SIMPLY BEAUTIFUL Rewards Points.

REDEEMING SIMPLY BEAUTIFUL REWARDS POINTS IS EASY:
You need a minimum of 2,000 points to redeem your SIMPLY BEAUTIFUL Rewards. Here’s what the points are worth:

2,000 points   $   100
3,000 points   $   150
5,000 points   $   250
7,500 points   $   375
10,000 points $   500
15,000 points $   750
20,000 points $1,000

Once you’ve reached the 2000 point level, you may use your points toward the service of your choice. You do not need to have the full amount of points to cover the service; points can be put toward any treatment and you can simply pay the balance. Your points will be deducted at the time of your treatment, and our front desk staff will tell you your updated point balance (if you had more points than the service appointment was for) or tell you your new balance (if you were partially paying for a service with points).

Please keep in mind that our standard 24-hour cancellation policy remains in effect for all appointments.

SIMPLY BEAUTIFUL Rewards Points will expire after an eighteen month period.

SIMPLY BEAUTIFUL Rewards Points / The Fine Print
Members earn 1 SIMPLY BEAUTIFUL Reward Point for every $1 spent on any service or product at COSMETIC LASER MD (not including tax or spa gratuities, or fees).

SIMPLY BEAUTIFUL Rewards Points cannot be transferred to another individual.

SIMPLY BEAUTIFUL Rewards Points are non-transferable and have no cash value.

SIMPLY BEAUTIFUL Rewards Points will expire after an eighteen month period.

Gift Certificates purchased (and earning double point value) cannot be used by purchaser. No more than one $500 gift certificate can be purchased for any one person.

Points can be redeemed on any aesthetic and spa treatment; at this time SIMPLY BEAUTIFUL Rewards Points cannot be used to purchase TANNE MD SKIN CARE or makeup items.

Our standard 24-hour cancellation policy still applies to all appointments. If you must change or cancel an appointment, please do so with at least 24-hour minimum notice. Please remember that we put time aside especially for you and that we cannot re-schedule an opening on the same day. Please be respectful of our time, and give us a courtesy call if you cannot make your appointment.

COSMETIC LASER MD reserves the right to terminate SIMPLY BEAUTIFUL Rewards or to change these Terms and Conditions at any time with or without notice. COSMETIC LASER MD will have final decision should any disputes over SIMPLY BEAUTIFUL Rewards or the awarding of Rewards points arise.
